HTC Transition Resources
Transitioning from a pediatric Hemophilia Treatment Center (HTC) to an adult HTC is a critical step in managing your long-term health and independence. As you grow older, your medical needs, responsibilities, and lifestyle may change, and it’s important to ensure your care evolves with you. The transition process helps you take a more active role in your treatment, understand your bleeding disorder, and build relationships with a new care team that will support you into adulthood. Determining if you’re ready to begin this journey involves assessing your knowledge about your condition, your ability to manage treatment, and your comfort in making health-related decisions. Reviewing resources provided by your care team can help you better understand what to expect, what skills you’ll need, and how to prepare for a smooth and successful transition.
To help guide this process, the transition of care includes a three-part assessment that begins in early adolescence. The first phase, typically starting between ages 12–14, focuses on building awareness of your bleeding disorder, learning basic self-care skills, and beginning conversations about taking more responsibility for your health. The second phase, between ages 14–17, encourages deeper engagement, such as managing appointments, understanding insurance, and beginning to communicate directly with your care team. The final phase, starting at age 17 and beyond, is about preparing for a full transfer of care to an adult HTC. This includes ensuring you can independently manage your treatment plan, refill medications, advocate for yourself, and navigate adult healthcare systems. Each step is designed to support you in becoming confident and prepared for adult care.
